About the project
The Better Together Project at GSH is a peer support initiative designed to assist adolescents and young adults (ages 13-24) living with chronic health conditions. This programme fosters a safe, supportive community where participants can connect, share experiences, and improve their mental health and treatment adherence. Established in 2017, Better Together has already demonstrated significant positive outcomes and aims to expand its reach over the next three years.
Adolescents with chronic conditions such as HIV, renal disease, diabetes, and mental health disorders often experience stigma, isolation, and difficulty adhering to treatment. Transitioning from pediatric to adult care can be overwhelming, leading to decreased engagement in healthcare and poorer health outcomes. Without targeted support, these young individuals face heightened risks of mental health challenges, including depression and anxiety.
The Better Together Project operates weekly peer-led support groups, supervised by trained clinicians and social workers. Peer mentors—adolescents who have successfully navigated their conditions—offer guidance, hospital navigation support, and emotional encouragement. The initiative also includes caregiver support groups and outreach efforts to increase programme awareness and participation.
Help us raise R1.35 million for the next three years (2025-2028). This will allow the project to continue and expand its impactful work, ensuring every adolescent at GSH finds a welcoming and supportive community.
